    PATIENT REFERRAL POLICY

    1. Purpose

    This policy outlines the terms and conditions for patient referrals at Nutrition Solutions For Me (“NSFM,” “the Practice,” “we,” “us”), including the requirements that must be met for a referral to be considered valid.

    2. Definition of a New Patient

    For purposes of this policy, a “New Patient” is an individual who:
    • Has never received services from Nutrition Solutions For Me
    • Is not an established patient of the Practice
    • Has never scheduled, completed, or attended an appointment with the Practice
    Patients may refer friends or family members provided the individual meets all criteria above.

    3. Referral Submission & Timing Requirements

    To be eligible for referral consideration:
    • All referrals must be submitted through the official Nutrition Solutions For Me referral link
    • The referral form must be completed in full and accurately
    • The referral link must be submitted prior to the referred individual’s first scheduled appointment
    • Referrals submitted verbally, by email, or after an appointment has been scheduled or completed will not be accepted
    Failure to comply with these requirements will result in the referral being deemed invalid.

    4. Appointment Completion Requirement

    A referral is considered complete only after the referred New Patient completes at least one billable appointment with the Practice. A referral will not be considered complete if the referred individual:
    • No-shows
    • Cancels
    • Fails to complete the appointment

    5. Referral Processing and Confirmation

    • Referrals are processed using the information submitted through the official referral link
    • Referral status is confirmed after verification of the referred New Patient’s first completed appointment
    • Incomplete, inaccurate, or late submissions may be rejected at the Practice’s discretion

    6. Limitations and Exclusions

    • Referrals are one-time per New Patient
    • Referrals will not be accepted for:
      • Referring themselves
      • Referring an existing or previously scheduled patient

    7. Modification and Termination

    Nutrition Solutions For Me reserves the right to modify, suspend, or terminate this Patient Referral Policy at any time, with or without notice, at its sole discretion.
